Swanley deckman death report finds unsafe practices

An investigation into a fatal accident of a crew member on a Woolwich ferry has found “unseamanlike” working practices on board.

Ben Woollacott, from Swanley, died on August 3 2011 after falling off a River Thames passenger ferry while trying to unmoor it.

The 19-year-old suffered severe head injuries and did not regain consciousness after he was rescued from the water.

The Marine Accident Investigation Brance (MAIB) said there was a “lack of recognition [...] of the dangers associated with unmooring without supervision”.

“This situation was compounded by a lack of suitable oversight at the time of the accident.”

The MAIB said a number of control measures were being taken to prevent a similar accident in the future.
